Introduction
Collaboration among healthcare professionals is crucial for providing quality care to nursing home residents. Poor teamwork can lead to unnecessary hospital visits, which can be stressful and harmful for residents. The SaarPHIR project aimed to improve cooperation between general practitioners and nurses to reduce hospitalizations and enhance care quality.
Methods
A study was conducted in Saarland, Germany, from May 2019 to July 2020. It involved 28 nursing homes with over 1,000 residents. The study compared two groups: one that received the new care approach and one that continued with standard care. The main focus was on hospitalization rates, using data from health insurers to evaluate outcomes.
Results
The study found that hospitalization rates did not significantly change between the two groups. However, there was a notable reduction in nursing home care-sensitive admissions for those in the new care program. Costs for medications were higher in the intervention group, but costs for hospital stays were lower.
Conclusion
While the overall hospitalization rates did not show significant improvement, the intervention had some positive effects on specific admissions. Participation levels were lower than expected, which may affect the reliability of the findings.
