A group psychological intervention for postnatal depression in British mothers of South Asian origin — the ROSHNI-2 RCT

Background

Postnatal depression is more common among British South Asian women than white women in the UK. Cognitive-behavioral therapy is known to be effective, but there is limited evidence on its use in different ethnic groups.

Objectives

The study aimed to evaluate the effectiveness and cost of a culturally adapted group therapy called the Positive Health Programme for British South Asian women with postnatal depression, compared to standard treatment.

Study Details

The research was conducted in general practices and children’s centers across various regions in the UK. It involved British South Asian women aged 16 and older, with infants up to 12 months, who met the criteria for depression.

Intervention

The Positive Health Programme consisted of 12 sessions of group therapy based on cognitive-behavioral principles, led by trained facilitators.

Results

Out of 732 participants, nearly half in the Positive Health Programme group recovered from depression after 4 months, compared to 37% in the standard treatment group. By 12 months, recovery rates were similar in both groups, indicating a need for ongoing support.

Qualitative Feedback

Participants found the Positive Health Programme to be acceptable and beneficial, reporting improved emotional support and well-being.

Cost-Effectiveness

The average cost of the Positive Health Programme was £408 per participant. It was considered cost-effective, especially for those who attended more sessions, costing £9,040 per quality-adjusted life year.

Limitations

The study’s findings may not apply to other ethnic groups, and the cost analysis did not account for potential biases.

Conclusions

The culturally adapted therapy was effective and well-received by South Asian women experiencing postnatal depression.

Future Work

Further development and evaluation of the Positive Health Programme are needed, including longer follow-ups and exploring remote delivery options.
