Background
HIV transmission has risen in the U.S. among people who inject drugs. Testing for HIV and connecting individuals to treatment is essential for both personal and public health. However, many barriers prevent access to testing, even within programs designed to support this population.
Study Overview
This study will evaluate a new approach to improve HIV testing and referrals in syringe services programs (SSPs). We will conduct a trial from January 2023 to January 2025, involving 32 SSPs across the country.
Implementation Strategy
The strategy being tested is called the Systems Analysis and Improvement Approach (SAIA). This plan aims to enhance how SSPs provide HIV testing and connect individuals to prevention and treatment services, like pre-exposure prophylaxis (PrEP).
Trial Design
Participants will be divided into two groups: one will receive the SAIA approach, while the other will follow standard procedures. The SAIA group will work with trained specialists to:
- Map their processes to identify specific needs.
- Analyze and prioritize areas for improvement.
- Test solutions through ongoing quality improvements.
Data Collection
We will gather data on outcomes for 21 months, including a prep phase, an implementation phase, and a sustainment phase. We aim to measure the effectiveness of the SAIA approach, as well as its costs and benefits.
Potential Impact
If successful, this strategy could be adapted for other health issues beyond HIV in similar service programs.
Next Steps
To improve HIV testing and referrals, clinics should:
- Define clear goals for patient and clinic outcomes.
- Select AI tools that meet specific clinical needs.
- Implement changes gradually and monitor results closely.
