Bringing malaria diagnosis and treatment closer to the people: economic rationale for expanding malaria community case management to all ages in a rural district in Madagascar

Background

Expanding malaria community case management (mCCM) to all ages can improve access to healthcare in areas that need it the most. This study looked at how practical and cost-effective this change would be in the Farafangana district of Madagascar.

Study Methods

Researchers conducted a trial across 30 health centers involving 502 community health workers (CHWs). The intervention included training and resources to help manage malaria in people of all ages. They analyzed how much this program would cost and how effective it was in saving lives and improving health outcomes.

Results

The total cost for the program, without paying CHWs, was $794,000 per year, mainly for malaria treatments and diagnostics. The study found that for every disability-adjusted life year (DALY) averted, costs ranged from -$21.86 to $212.42. It was determined that the program could save lives—preventing about 99.6 deaths and yielding significant financial benefits of $1,172,283 annually. Overall, it showed that this approach is cost-effective and beneficial for communities.

Conclusions

Expanding mCCM to include all ages is a smart, cost-effective way to improve malaria treatment in low-resource areas.
