This study explored how combining different local anesthetics can improve pain relief after arthroscopic shoulder surgery.
Background
After shoulder surgery, many patients experience acute pain. While supraclavicular brachial plexus block helps manage this pain, its effects don’t last long enough. This study aimed to find out if combining various local anesthetic adjuncts could extend pain relief.
Methods
The trial involved 80 patients who were divided into four groups:
- Group DMD: Dexamethasone, ropivacaine, dexmedetomidine, and magnesium sulfate
- Group DM: Ropivacaine, dexmedetomidine, and magnesium sulfate
- Group M: Ropivacaine and magnesium sulfate
- Group D: Ropivacaine and dexmedetomidine
The main goal was to measure how long it took for patients to request pain relief after surgery. Secondary goals included tracking opioid use and pain scores at various times post-surgery.
Results
Patients in Group DMD experienced:
- Significantly longer time before requesting pain relief compared to Group DM and Group M.
- Lower opioid consumption at 18 and 24 hours after surgery compared to Group DM and Group M.
- Lower pain scores at 6 and 12 hours post-surgery compared to other groups.
- Better overall shoulder function scores than Groups DM and D.
Conclusions
Combining dexamethasone, magnesium sulfate, and dexmedetomidine with ropivacaine offers better pain relief than using magnesium sulfate alone or in combination with dexmedetomidine.
