Effect of Cuff Inflation with Lidocaine, Saline, and Air on Tracheal Tube Cuff Pressure during Laparoscopic Resection of Colorectal Neoplasms: A Randomized Clinical Trial

Background: This trial aimed to determine if inflating tracheal tube cuffs with normal saline or lidocaine can prevent an increase in cuff pressure and tracheal mucosal damage during laparoscopic surgeries with general anesthesia. Additionally, the study sought to assess the predictive value of changes in tracheal tube cuff transverse diameter for changes in cuff pressure.
Methods: Ninety patients undergoing laparoscopic resection of colorectal neoplasms were randomly assigned to groups inflating cuffs with air, saline, or lidocaine. Cuff pressure was monitored and adjusted, and changes in cuff pressure and tracheal mucosal injury were recorded.
Results: Tracheal tube cuff pressure did not significantly differ among the groups. Changes in tracheal tube cuff transverse diameter were found to predict cuff pressure. Tracheal mucosal injury was lower in the lidocaine group compared to the air and saline groups.
Conclusions: While inflation with normal saline and 2% lidocaine did not prevent an increase in cuff pressure during laparoscopic surgeries, lidocaine was associated with reduced postoperative tracheal mucosal injury. Changes in tracheal tube cuff transverse diameter, measured by ultrasound, were predictive of cuff pressure changes.
