Effects of Speculum Lubrication on Cervical Smears for Cervical Cancer Screening: A Double Blind Randomized Clinical Trial

Background

Speculum lubrication may reduce pain during Pap-smear collection, potentially increasing uptake of cervical cancer screening. However, concerns exist about its impact on cytological results.

Aim

To compare the adequacy of cervical cytology smears and pain scores of women undergoing cervical cancer screening with or without speculum lubrication.

Methods

A randomized controlled study of 132 women undergoing cervical cancer screening at a Nigerian tertiary hospital. Participants were assigned to ‘Gel’ or ‘No Gel’ groups. Pap smears were collected with a lubricated or non-lubricated speculum. Outcome measures included unsatisfactory cervical cytology smears, pain scores, willingness for repeat testing, and cytological diagnosis.

Results

No significant difference in unsatisfactory cervical smear results between the groups (13.6% vs. 21.2%, p = 0.359). Mean pain scores were significantly lower in the gel group (45.04 vs. 87.96; p<0.001). An equal proportion of participants in each group were willing to come for repeat testing (90.9% vs. 90.9%; p > 0.999).

Conclusion

Speculum lubrication did not affect the adequacy of cervical smears but significantly reduced pain during pap smear collection. It also did not significantly affect willingness for repeat testing.
