Emotional Processing Following Digital Cognitive Behavioral Therapy for Insomnia in People With Depressive Symptoms: A Randomized Clinical Trial

Importance of Cognitive Behavioral Therapy for Insomnia (CBT-I)

CBT-I can help reduce symptoms of depression, but we need to understand how it works better.

Study Objective

The goal of this study was to see if CBT-I changes the way people perceive emotions in facial expressions and if these changes help improve depressive symptoms.

Study Design

We conducted a clinical trial comparing digital CBT-I with sleep hygiene education. Adults in the UK with insomnia and depression were recruited online and randomly assigned to either a 6-session CBT-I program or a webpage about sleep hygiene. The study took place from April 2021 to January 2022, with follow-ups at 5 and 10 weeks.

Main Outcomes

We measured how accurately participants recognized happy and sad facial expressions, along with other factors like insomnia severity and depressive symptoms.

Results

A total of 205 participants took part, with 101 in the CBT-I group and 104 in the control group. Most participants were women, and the average age was about 49 years. At 10 weeks, CBT-I significantly reduced insomnia severity and depressive symptoms compared to sleep hygiene education. Improvements were noted in:

  • Insomnia severity
  • Depressive symptoms
  • Negative emotions
  • Emotional regulation
  • Worry
  • Perseverative thinking

Additionally, CBT-I increased positive emotions. Changes in negative emotions, emotional difficulties, and worry helped explain why depression symptoms improved over time.

Conclusions

The study did not find that CBT-I changed how people perceive facial expressions. However, it did improve insomnia and depressive symptoms. The early changes in negative emotions and worry are important and require further study.
