Ketamine vs Electroconvulsive Therapy for Treatment-Resistant Depression: A Secondary Analysis of a Randomized Clinical Trial

The ELEKT-D trial demonstrated that intravenous ketamine was noninferior to ECT for nonpsychotic TRD. This secondary analysis aimed to identify clinical features guiding the selection of ketamine vs ECT.
MAIN OUTCOMES AND MEASURES
The study found that outpatients with moderately severe or severe depression may consider ketamine over ECT for TRD, as they showed greater improvement with ketamine treatment.
CONCLUSIONS AND RELEVANCE
These findings suggest that intravenous ketamine may be a preferable option for certain patient groups with TRD, based on the observed greater improvement in depression among outpatients with nonpsychotic TRD who had moderately severe or severe depression. This could inform shared decision-making for patients with TRD.
