Importance: Poor medication adherence in patients with Chronic Heart Failure (CHF) can lead to serious health issues, especially in China.

Objective:

To evaluate whether a pharmacist-led management model using a social media platform improves medication adherence compared to regular care.

Study Details:

This study was a randomized clinical trial conducted from March 2021 to May 2023 in five hospitals in China, involving 445 participants over a 52-week follow-up.

Participants were 18 years or older with a CHF diagnosis and stable medication.
They were randomly divided into two groups: one received pharmacist-led management, and the other received usual care.

Intervention:

The intervention group had monthly follow-ups with pharmacists and used the WeChat app for communication and education. The control group had standard follow-ups with nurses.

Main Outcomes:

The main measure of success was the proportion of days patients took their heart failure medication correctly over 52 weeks.

Results:

In the study of 445 patients, those in the pharmacist-led group showed:

An 8.1% higher medication adherence rate.
A significant increase in patients adhering to medication 80% of the time or more.

Conclusion:

This trial showed that pharmacist-led management can enhance medication adherence in CHF patients compared to usual care.
