This study was a randomized clinical trial conducted from March 2021 to May 2023 in five hospitals in China, involving 445 participants over a 52-week follow-up.
- Participants were 18 years or older with a CHF diagnosis and stable medication.

Intervention:
The intervention group had monthly follow-ups with pharmacists and used the WeChat app for communication and education. The control group had standard follow-ups with nurses.

The main measure of success was the proportion of days patients took their heart failure medication correctly over 52 weeks.
Results:
In the study of 445 patients, those in the pharmacist-led group showed:
- An 8.1% higher medication adherence rate.
- A significant increase in patients adhering to medication 80% of the time or more.

This trial showed that pharmacist-led management can enhance medication adherence in CHF patients compared to usual care.
