The Impact of Back Optic Zone Design in Orthokeratology on Visual Performance

ABSTRACT

PURPOSE: This study aimed to assess the visual performance in adolescents undergoing orthokeratology treatment with different optical zone diameters (OZDs).

METHODS: The study was conducted at Tianjin Eye Hospital in June 2022, enrolling thirty-six schoolchildren. They were fitted with corneal refractive therapy lenses with two sizes of OZDs (5 mm and 6 mm) and were randomized to wear different sizes in each eye. Visual quality was evaluated subjectively using visual acuity, refraction, contrast sensitivity, and visual symptoms, and objectively using ocular higher order aberrations and modulation transfer function.

RESULTS: Thirty-five myopic children completed the 1-month follow-up. The 5 mm OZD induced smaller treatment zone diameters than the 6 mm OZD. Subjective visual quality did not show significant differences between the two groups. Some aberrations increased significantly from baseline in both groups. Spherical aberration in the 5 mm OZD group was significantly higher for 3-mm pupils. The 6 mm OZD group showed significantly higher MTF values for certain cycles for 3-mm pupils. However, there were no significant differences between the two groups for 5-mm pupils.

CONCLUSIONS: The difference induced by varying OZD was significant only in the smaller pupil condition, suggesting that OZD selection for OrthoK designs should consider individual pupil size in patient management.

TRANSLATIONAL RELEVANCE: This study highlighted that the objective visual quality of small OZD lenses was only slightly affected for small pupil size.
