Comparison of the Effects of Adipose Extracellular Matrix/Stromal Vascular Fraction Gel Injection and CO2 Fractional Laser on Atrophic Acne Scar in Asians Through a 24-Week Prospective, Randomized, Split-Face Study

Background

Adipose extracellular matrix/stromal vascular fraction gel (ECM/SVF-gel) is a treatment that includes stem cells and other components from fat tissue. It helps produce collagen and can fill in scars.

Aim

This study aimed to compare the effectiveness and safety of ECM/SVF-gel injections for treating acne scars with CO2 fractional laser (CO2FL) treatments.

Methods

The study was conducted at Xijing Hospital in China from July 2020 to December 2022. Patients with moderate to severe acne scars received either an ECM/SVF-gel injection on one side of their face or two CO2FL treatments on the other side. The main measure of success was the change in acne scar scores after 24 weeks. We also looked at changes in scar volume using 3D imaging software.

Results

Eleven participants joined the study, and ten completed it. The average age was 27.5 years, with a mix of male and female participants. After 24 weeks:

  • The ECM/SVF-gel side showed a greater improvement in scar scores, with an average change of -60.25, compared to -43.25 for the CO2FL side.
  • The volume of scars decreased by 33.17% on the ECM/SVF-gel side, while it decreased by 19.69% on the CO2FL side.

Conclusions

ECM/SVF-gel injections are a safe and effective option for treating acne scars.
