Sudden Gains in Treating Prolonged Grief in Children and Adolescents
Objective
Sudden gains refer to significant and lasting improvements in symptoms that occur between two treatment sessions. This study looks at sudden gains in children and adolescents with Prolonged Grief Disorder (PGD), a condition recognized in recent mental health classifications. The goal was to explore sudden gains during Cognitive Behavioral Therapy (CBT) and supportive counseling for those showing PGD symptoms.
Method
The study involved 118 patients (50% female, average age 12.93 years). They were divided to receive either nine sessions of CBT or supportive counseling. PGD symptoms were tracked using a Grief Checklist. Sudden gains were identified based on established criteria, and treatment outcomes were compared between those who experienced sudden gains and those who did not.
Results
Only 9 patients (7.63%) showed sudden gains, primarily during CBT. There were no significant differences in PGD, PTSD, or depressive symptoms between the two groups, but those with sudden gains showed improvement in problem behaviors at follow-up assessments.
Conclusions
Sudden gains are rare and have limited clinical importance in treating PGD in young people. More research is needed to understand what predicts sudden gains and other symptom changes during PGD treatment.
