Acceptability and Effectiveness of a Fully Web-Based Nutrition and Exercise Program for Individuals With Chronic Disease During COVID-19: Randomized Controlled Trial

Background

In-person nutrition and exercise programs help improve health for people with chronic diseases. However, we need to understand how well online programs work, especially with different levels of support.

Objective

This study evaluates a fully online nutrition and exercise program from start to finish.

Methods

We conducted a controlled trial using the Heal-Me platform. Participants included adults with a history of cancer, lung disease, or transplants who had internet access and had previously joined a rehabilitation program. They were divided into three groups:

  • Control Group: Received a self-guided nutrition and exercise guide.
  • Heal-Me Light Group: Received the guide plus group classes led by dietitians and exercise specialists.
  • Heal-Me Intensive Group: Received everything from the Light group plus one-on-one sessions with specialists.

All participants wore activity trackers. We measured how well they adhered to the program and their improvement in physical function.

Results

Out of 216 participants, 202 completed the program, showing excellent adherence, with 82% attending over 75% of the activities. Participants rated the program very positively, with similar satisfaction across both Heal-Me groups. There were no significant differences in physical function improvement among the groups, but those in the Heal-Me groups showed benefits in other areas, like well-being and energy levels. The Heal-Me Intensive group reported better self-management of nutrition.

Conclusions

Overall, the online nutrition and exercise program was well-received and effective for individuals with chronic diseases. Future studies could refine the program by using more precise measures of physical function and increasing exercise intensity for experienced participants.
