Effect of Hospital-to-Home Transitional Care for COPD on Patient-Centered Outcomes
Background
Transitional care from the hospital to home has been shown to improve healthcare use and health outcomes for patients with COPD. However, research on its impact on patient-centered outcomes, like symptoms and experiences, is limited.
Study Overview
This study was a single-blind randomized controlled trial conducted in South Korea with 179 COPD patients. There were two groups: one receiving transitional care (87 patients) and the other receiving usual care (92 patients). The transitional care included:
- Post-discharge care planning
- Personalized education
- Breathing exercises
- Telephone counseling
- Home visits
- Referral to social services
Results
The results showed that patients receiving transitional care had:
- Better breathing symptoms (Modified Medical Research Council scores: 1.3 vs. 1.82, P = .002)
- Lower COPD Assessment Test scores (6.32 vs. 9.43, P = .001)
- Increased inhaler usage (49.69 vs. 46.86, P = .002)
- Lower depression and anxiety scores
- Better experiences during hospitalization (39.34 vs. 37.5, P = .036)
- Greater preparedness before discharge (34.54 vs. 32.3, P = .003)
- Improved post-discharge management (34.72 vs. 30.29, P = .003)
Conclusions
Transitional care services can significantly improve patient-centered outcomes. Our findings support the importance of implementing patient-centered transitional care for COPD patients.
