The Effect of Vildagliptin vs. Metformin on Liver Fat in Type 2 Diabetes Patients

This study investigated how two medications, vildagliptin and metformin, affect liver fat in patients recently diagnosed with type 2 diabetes (T2D). We used the Hepatic Steatosis Index (HSI) and ultrasound to measure liver health.

- Participants: 246 patients with newly diagnosed T2D.
- Groups:
- Group 1: 117 patients received 50 mg of vildagliptin twice daily.
- Group 2: 129 patients received 500 mg of metformin twice daily (can increase to 2000 mg).
- Assessments: Fasting blood sugar, HbA1c, weight, body mass index (BMI), waist and hip measurements, HSI, and ultrasound grading at the start and after 6 months.
Results
Both medications showed significant improvements in blood sugar levels, weight, BMI, and liver fat over 6 months (p < 0.001). However, metformin led to greater weight and BMI loss compared to vildagliptin (p = 0.001 and p = 0.009 respectively). There was no significant difference between the two in liver fat improvement via ultrasound.
