A Protocol for a Nested Randomised Controlled Trial to Evaluate the Effect of a Pathway Navigation Intervention on Lung Cancer Screening Uptake

Introduction: Lung cancer is a leading cause of cancer death worldwide. The UK National Screening Committee recommended a national targeted lung cancer screening programme in 2022 to improve early diagnosis and survival rates. However, there are socioeconomic and smoking-related disparities in screening uptake. This trial aims to assess whether a pathway navigation intervention can increase participation in lung cancer screening.
Methods and Analysis: The trial involves a two-arm randomised controlled trial and process evaluation within the YLST. Participants aged 55-80 who have not responded to previous postal invitations to screening will be randomised to receive pathway navigation or usual care. The pathway navigation intervention includes a postal appointment notification and prearranged telephone appointment with a trained navigator to guide participants through the screening process.
Primary Outcomes: The number undergoing initial telephone assessment of lung cancer risk and the number undergoing a lung cancer screening scan. Secondary outcomes include demographic, clinical, and risk parameters of participants undergoing the assessment, as well as the number of screen-detected cancers diagnosed and costs.
Ethics and Dissemination: The study is approved by the Greater Manchester West Research Ethics Committee and the Health Research Authority. Results will be shared through peer-reviewed scientific journals, conference presentations, and the YLST website.
