The influence of an interactive educational approach on advance care planning counseling in individuals with psychiatric disorders

Background

In Taiwan, patients with psychiatric conditions often face unfair judgments that affect their ability to make healthcare decisions. These judgments may arise from concerns about their cognitive abilities, emotional stability, and communication skills. As a result, their independence in making healthcare choices is often compromised. There is a significant lack of understanding and advocacy for Advance Care Planning (ACP) among these individuals. This research aims to see how an interactive educational approach can help psychiatric patients understand ACP better and improve their attitudes and intentions regarding Advance Decisions (AD).

Methods

We recruited 90 participants for our study, dividing them into two groups using random selection. One group continued with regular self-care education, while the other group attended additional interactive educational sessions every two weeks for three months. This intervention included six one-hour group sessions. We assessed the participants’ knowledge, attitudes, and willingness to sign Advance Decisions before and after the intervention.

Results

Both groups were similar at the start of the study. After the intervention, the group that received the interactive education showed a significant improvement in knowledge (3.31 points increase) and attitudes (4.91 points increase). However, their intent to sign an Advance Decision did not show a significant increase (0.73 points increase).

Conclusions

The interactive educational approach helped improve psychiatric patients’ knowledge and attitudes toward ACP, but it did not significantly change their willingness to sign Advance Decisions. We recommend empowering these patients to engage in ACP discussions. This study demonstrates that integrating ACP education into regular psychiatric care is feasible and beneficial.
