Impact of a Community Pharmacy-Based Medication Therapy Management Program on Clinical and Humanistic Outcomes in Patients with Uncontrolled Diabetes: A Randomised Controlled Trial
Abstract
This study aimed to assess the impact of a community pharmacy (CP)-based medication therapy management (MTM) program on the health and satisfaction of patients with uncontrolled diabetes. Patients were randomly assigned to either the MTM program provided by pharmacists or standard care. The primary outcome was the change in HbA1c over 6 months. Secondary outcomes included changes in clinical parameters, types of drug-related problems, health service utilization, adherence, diabetes distress, and patient satisfaction with the service at 6 months.
A total of 160 participants were recruited, with the majority being male and having diabetes for more than eight years. The study found that the MTM program did not significantly lower HbA1c levels compared to standard care, although there was significant improvement within each arm. The intervention arm also demonstrated improvement in blood pressure control and showed a significant decrease in drug-related problems. Patient satisfaction and adherence were significantly higher in the MTM program group. The MTM program was found to prevent hospitalizations in patients with diabetes.
