Background
Less than 25% of people with borderline personality disorder (BPD) get the recommended therapy. Digital tools can help close this gap. This study tested a digital therapy called priovi for BPD.
Methods
We ran a study in Germany with patients diagnosed with moderate to severe BPD. Participants were at least 18 years old, had a safety plan for crises, and agreed to join the study. Those with substance use or psychotic disorders were not included. Patients were randomly assigned to either use the priovi digital therapy along with their usual treatment or just their usual treatment. The study was designed to keep researchers unaware of which group each patient was in until after the results were analyzed. All patients had access to standard care, and the control group received self-help materials for BPD. We measured the effectiveness of the therapy after 3 months using a specific symptom checklist and tracked any serious side effects through online surveys.
Findings
From May to October 2022, we screened 1,766 patients and included 580 in the study. At the 3-month mark, 12% of the priovi group and 5% of the control group dropped out. The average age of participants was 29 years. Our analysis showed that those using priovi had better outcomes than those who did not (p=0.0005). Importantly, there were fewer suicide attempts in the priovi group (7 attempts) compared to the control group (21 attempts), indicating a safer option (p=0.0081).
Interpretation
This study suggests that the priovi digital therapy is effective and does not raise safety concerns for people with BPD.
